Documents for Case Managers
Farmer Information Sheet (PDF) This two-page document (which can also be printed front-and-back) describes the benefits of Cell Count Solutions for farmers. It outlines the program from a farmer’s perspective, including an example of a farm that has successfully completed the program and is now producing low cell count milk for their dairy processor. Factory field staff and advisers use this sheet to illustrate their discussions with farmers considering the program. Benefits of Lower Cell Counts (PDF) Farmer Agreement (PDF) 11/09/06 This sheet documents the details of agreements between dairy companies and individual farmers. It is intended as a memorandum of understanding rather than a legally binding contract. Companies are encouraged to use this sheet to ensure both parties clearly understand the terms of participation in the program and their agreed roles and responsibilities. Permission To Contact Form (PDF) Monitoring of progress by case managers is most easily done when a herd’s cell counts can be forwarded directly to the adviser electronically. The permissions given in this form enables dairy companies to forward cell counts to case managers (preferably by electronic transfer of data), case managers to provide a summary of the mastitis investigation to dairy companies, and Countdown to evaluate the initiative (through BMCC changes and the opportunity for direct contact with farmers). Field staff are encouraged to explain the reasons behind each permission and ask the participating farmers to check the boxes (as they feel appropriate) and sign the form. Countdown Downunder Mastitis Investigation Pack (PDF) The Countdown Downunder Mastitis Investigation Pack provides a systematic and comprehensive approach to mastitis problem solving and action plan development. As such it is the fundamental resource for the advisory team in Cell Count Solutions investigations. Cell Count Solutions Manual for Dairy Companies and Advisers (PDF) The manual outlines the Cell Count Solutions initiative, describes the key steps and resources involved in taking a farm through the program, and the roles of the various advisers and the farm staff. The manual provides a reference for dairy company field staff and advisers on to how the program runs. Client Register (Excel) The client register is an Excel spreadsheet designed to enable those managing the program to keep a summary of who’s been offered the initiative, when they ‘signed on’, and when the investigation was completed and reviewed. Dairy company staff, case managers and Countdown Regional Project Managers are encouraged to use the register to keep track of farms involved in the program. Action Planning Review (PDF) A key learning from Countdown has been to understand what makes an action plan “do-able” on farm, and why they sometimes fail. The Action Planning Review presented here helps advisers to assess a mastitis action plan devised by themselves and the farmer. It is designed to help advisers identify risks to successful implementation of the action plan on the farm. Advisers can use the review sheets to identify the key areas of risk and suggest ways of overcoming those risks that are within the control of the farm team.
